What the trials found, who may benefit, and how microneedling compares with proven treatments.
Microneedling has moved from skincare into hair loss, and the claims range from miracle cure to waste of money. The research sits between the two. The strongest results come from combining microneedling with a hair growth medicine such as minoxidil, while microneedling on its own appears to have a limited effect.
At DSI Hair in Algonquin, Illinois, our dermatologists start by finding the cause of your hair loss. Here is what the evidence says about microneedling, who might benefit, and where it fits next to treatments with stronger track records.
The Short Answer: It Helps Most as an Add-On
A 2024 meta-analysis of 13 randomized trials found that combining microneedling with other hair growth therapies significantly increased hair density and hair diameter in androgenetic alopecia compared with the other therapy alone, without significant adverse effects. A 2025 meta-analysis of microneedling plus minoxidil reached a similar conclusion for 5% minoxidil, and also concluded that microneedling by itself may have only a limited effect.
The best-supported use of microneedling is as a partner to a proven treatment, not as a replacement for one.
How Microneedling Is Thought to Work
Microneedling creates many tiny, controlled injuries in the scalp. The leading explanations are that the healing response releases growth factors that wake up follicles, and that the tiny channels may help topical medicines such as minoxidil reach the skin. These are proposed mechanisms, and researchers are still testing which one matters most.
What the Research Shows
| Approach | What the studies found |
|---|---|
| Microneedling plus minoxidil | Higher hair count and hair diameter than minoxidil alone in pooled randomized trials. |
| Microneedling plus other treatments (such as PRP, finasteride, or growth factor solutions) | The 2024 meta-analysis pooled these combinations and found increased hair density and diameter compared with the other treatment alone. |
| Microneedling alone | A limited effect compared with combination treatment. The evidence is weaker. |
One of the best-known trials, by Dhurat and colleagues in 2013, randomly assigned 100 patients to minoxidil alone or minoxidil plus microneedling. As reported in the 2024 meta-analysis, hair count was significantly higher in the combination group.
Limits of the evidence
- The trials are small and results vary a lot between them, so the size of the benefit is uncertain.
- Needle depth, session frequency, and the products used differ from study to study, and there is no agreed standard protocol.
- Most trials follow patients for a few months, so long-term results are less clear.
Who May Benefit From Microneedling
Microneedling is most relevant for early to moderate androgenetic alopecia, where follicles are shrinking but still alive, and for patients who already use minoxidil and want to add something. It cannot bring back follicles that are gone, and it is not a treatment for scarring alopecia. A scalp exam is the way to find out which type of hair loss you have, which is why diagnosis comes before any procedure.
In-Office Microneedling vs. At-Home Derma Rollers
At-home rollers and pens vary widely in needle length and quality, and they are hard to keep clean. Problems can include irritation, infection, and bleeding when needles are too long or technique is poor. If you are considering microneedling, ask your dermatologist about needle length, how often to treat, and when to apply topical products afterward, because timing can change how the skin reacts.
Where Microneedling Fits Among Proven Treatments
For most patients with pattern hair loss, treatment starts with options that have the strongest evidence and regulatory approval, such as minoxidil, oral minoxidil, and, for men, finasteride. Procedures and devices such as PRP hair restoration and the Revian laser cap are added when a plan calls for them. Our guides to whether PRP works for hair loss and PRP for thinning hair explain how those options compare.
Safety and Side Effects
In the pooled trials, microneedling combinations did not cause significant adverse effects. Expected effects are temporary redness, tenderness, and minor bleeding. Tell your provider if you have an active scalp infection or inflammation, a bleeding disorder, or take blood thinners before any needling procedure.
How Long Before You Would See Results
Hair growth is slow. Many trials report outcomes at around three to six months, and hair that is still growing from existing follicles takes time to thicken. If you try any hair loss treatment, judge it over months, not weeks. Our guide to how long minoxidil takes to work gives a month-by-month picture that applies to most medical treatments.

Frequently Asked Questions: Microneedling for Hair Loss
Does microneedling regrow hair on its own?
The evidence for microneedling alone is limited. The stronger evidence is for combining it with minoxidil or another hair growth treatment.
Can I use a derma roller at home for hair loss?
Home devices are not the same as the controlled treatments used in studies. Needle length, hygiene, and technique all affect safety. Talk to a dermatologist before trying one.
How often is microneedling done for hair loss?
Protocols vary widely between studies and clinics, and there is no single standard. Your dermatologist can tell you what makes sense for your scalp if microneedling is part of your plan.
Is microneedling better than PRP for hair loss?
They are different treatments with different evidence. Both have randomized-trial support, mostly as part of a combination plan. A dermatologist can help you decide whether either fits your type of hair loss.
